If you're involved in a guardianship proceeding through the Nassau County Surrogate's Court, you'll need to complete a background investigation — and that means getting your fingerprints submitted to New York State. This guide walks you through the entire process so you know exactly what to expect, where to go, and what to bring.
Why Are Fingerprints Required?
The Nassau County Surrogate's Court requires a background investigation as part of every guardianship proceeding. As part of that process, your fingerprints must be submitted to New York State for their report. Any report received by the Court is kept strictly confidential.
New York State requires you to obtain your fingerprints digitally through a company called IdentoGO. Electronic fingerprint services are provided by IdentoGO by IDEMIA.
Information You Need to Provide to IdentoGO
When scheduling your appointment and visiting an IdentoGO location, you'll need to provide the following information:
- ORI (Originating Agency Identifier) for Nassau Surrogate's Court: NY029SC3J
- Fingerprint Reason: Guardianship
- Two (2) forms of identification — at least one must have a photograph (check with IdentoGO for acceptable forms of identification)
- Surrogate's Court File Number assigned to your proceeding — a petition must be filed with the Court to obtain a file number prior to obtaining fingerprints
Scheduling Your Appointment
It is your obligation to make an appointment to have your fingerprints taken by IdentoGO by IDEMIA. The Court does not make the appointment for you.

- If you schedule your appointment through the website, it is recommended that you print out the confirmation and bring it with you to the appointment
- You will be charged a fee by IdentoGO for the processing of your fingerprint
After Your Fingerprints Are Taken
IdentoGO will provide you with two (2) receipts:
- One for your personal records
- One to be filed with the court
Important: You must file a receipt from IdentoGO with the Surrogate's Court in order for the Court to review the Fingerprint Report.
